How to Enroll Dental and Vision Benefits for Individuals

How Do I enroll as an individual to the dental and vision plan?

Step one: Complete application http://www.strategynw.com/pdfs/Ameritas_Enrollment_indiv.pdf. (Group enrollment paperwork is different, please call 503-667-0500)

Step two: Authorize an Automatic Bank Transfer (EFT Form http://www.strategynw.com/pdfs/Debit_authoriz.pdf) Paper billing is $1 per employee per month additional

Step three: Mail paperwork with first months check to the following address:
